Updates of the list of DDDs for combined products

Newly assigned DDDs for combinations have been included in the list.
In order to improve the information regarding assignment of DDDs for fixed combinations in ATC groups C02, C03, C07, C08 and C09, general rules on how to assign DDDs in these groups have also been included.
